Zoey’s Grant Application Guidelines

Zoey’s Four Legged Friends Foundation

Please fill out the form below completely. Once the application is submitted, you will receive a confirmation that it has been submitted within 24 hours. If you do not receive a confirmation in the allotted time or have any questions on the application, please email us at zoeysfourleggedfriends@gmail.com. This Grant only applies to Emergency vet care that will save the pet from Euthanasia. There is only a one-time Grant per pet in the household allowed.


APPLICATION GUIDELINES

  1. The vet must speak with a member of the Zoey’s Four Legged Friends Foundation.

  2. The animal must be up to date on rabies vaccine and distemper/parvo vaccines. 

  3. The animal must be spayed or neutered.

    1. If the pet is under 18 months old, this rule does not apply.

    2. If the pet is over the age, the spay or neuter must be done at the owner’s expense. The vet must approve that the spay or neuter can be done. 

  4. The owner must be in good financial standing with the vet clinic. 

  5. The pet must be in overall good health and taken care of by the owner.

  6. Number of pets in the home will be a qualifier.

  7. This Grant does not apply to long term medical issues such as seizures, diabetes, blood disorders, long term cancer care, etc. 

  8. This Grant pays 100% of the emergency surgery.  

  9. If there are complications after the surgery and the pet needs to stay longer than expected, including medications, will be decided on a case by case basis.

  10. Once the pet leaves the pet hospital and needs medication at home, that is the owner’s financial responsibility. 

  11. Any follow up visits and/or continued medication is the owner’s financial responsibility.

  12. Before and after pictures are required to be used and become the property of Zoey’ Four Legged Friends website. The owner can remain anonymous if they would like. We would encourage positive testimonials on our website. 

We appreciate your understanding that Zoey’s Four Legged Friends is not able to help all cases due to limited funding. We will take every case with serious consideration, evaluating the severity and outcome of the case with all vet(s) involved. Any false information or misinformation about the animal’s case will result in the immediate denial of any funding now and in the future. Zoey’s Four Legged Friends is not responsible for any injury, complications or death during surgery or in the aftercare of the pet.

QUESTIONS & ANSWERS 

  1. Zoey’s Four Legged Friends (ZFLF) does not pay for any routine vet care, blood work, heart worm testing, flea and tick preventive, grooming, spay/neutering or boarding. 

  2. ZFLF does not fund any reoccurring vet visits. 

  3. The pet must reside in the home of the owner and must reside in the state of Missouri.

  4. The treating veterinarian must have assessed all injuries and has predicted the pet has a high prognosis of living a good quality of life. 

  5. The dog has never shown any aggression towards animals or humans in stressful situations; (Being at a vet clinic, unknown person in home, etc)

  6. The pet must not be terminally ill.

  7. ZFLF will pay 100% of the medical bills while in the hospital under veterinary care. 

  8. The owner is responsible for any medication, follow up visits after the pet has been released from the hospital. 

  9. Applications must include information directly from the veterinarian. 

    1. Diagnosis/Prognosis 

    2. Treatment cost

    3. History of the pet

    4. Proof of spay/neuter and vaccinations

  10. ZFLF will only release funds to the vet clinic once all requirements have been met. 

  11. ZFLF does NOT work with shelter or rescue groups.  

  12. If you have adopted a pet from a shelter in recent days of the pet’s injury, that will be decided by a board member and the veterinarian involved.

  14. If you believe your pet qualifies per the guidelines set forth, please download the application and a board member will be in contact with you within 24hours. 

  15. If ZFLF has questions, please make sure your contact information is correct. Someone will be in touch with you within the 24hour time period. 

  16. Any misrepresentation or false facts on the application will immediately close your case. You will never be able to apply for aid in the future with ZFLF. 

  17.  If your case is accepted, you agree for ZFLF to contact you periodically over the next year from the start date of the case. This is just to see how your pet is doing after the emergency care. 

  18. If your case is not accepted, ZFLF will notify you by email or phone.

  19. The owner must consent for any medical care the vet will do.
